Actual date on which the first participant was enrolled.This study focuses on helping people with Multiple Sclerosis (MS) improve their balance and walking abilities. MS often leads to muscle weakness and trouble with balance due to nerve damage in the central nervous system. This can significantly affect a person's quality of life, making daily activities challenging. The study aims to see if adding Cognitive Behavioral Therapy (CBT) to Pilates-based core stability training can offer better improvements in balance and walking than Pilates alone. This is important because improving core stability might lead to better control over body movements and reduce symptoms like fatigue, which is common in MS. Participants in the study are divided into two groups. One group will receive both Cognitive Behavioral Therapy and Pilates exercises, while the other group will only do the Pilates exercises. The Pilates exercises focus on strengthening core muscles, which are crucial for maintaining stability and proper posture. The treatment lasts for eight weeks, and researchers will observe how well each approach helps with balance and walking. By comparing the two groups, the study aims to determine whether the combination of therapies offers more benefits, potentially leading to better treatment strategies for people with MS.

Criteria
Inclusion Criteria: * A score of 3.5 or less on the Expanded Disability Status Scale (EDSS) because individuals with an EDSS score greater than 3.5 need more assistance, being less independent. * Age over 18 years * All definite multiple sclerosis patients diagnosed as MS according to revised McDonald criteria 2017 * Both males and females were included Exclusion Criteria: -Individuals who had orthopedic, neurologic, and psychological problems that accompanied MS that might affect the treatment results- * serious cognitive problems; performed regular exercise; were involved in another physiotherapy and rehabilitation program related to MS(Kurtzke, 1993) * Pregnant were not included. * Individuals who had an exacerbation during the treatment period
