This study aims to improve our understanding of pulmonary hypertension, a condition affecting the blood vessels in the lungs, and right heart dysfunction. It focuses on analyzing patients based on their molecular and clinical features, as well as other detailed characteristics like blood flow and imaging results. The goal is to create new ways to classify these conditions by linking these detailed descriptions with specific molecular causes. This could help identify different types of lung vascular diseases and find new markers that could aid in early diagnosis or in assessing how well treatments are working. Participants in this study will undergo evaluations every six months, which include clinical assessments, a 6-minute walk test, heart imaging, and blood tests for various molecular analyses. The study will compare these findings over time to see how well they predict disease progress, changes in symptoms, response to treatment, and survival. By doing this, the research aims to develop precise definitions of pulmonary vascular diseases using advanced molecular data, which could lead to better diagnosis and treatment options.

Criteria

5 inclusion criteria required to participate
Dialysis dependent renal function since the parent study acceptable

Patients ages \>18 years of age referred for right heart catheterization for further evaluation of known PVD or to be at risk for PVD due to established cardiac disease or pulmonary disease

Able to perform complete diagnostic testing listed subsequently (cardiac catheterization, echo, exercise test, PFT's, ECG, chest CT, quality of life questionnaires, ventilation/perfusion scan, cardiac MRI, body composition bioimpedance, and sleep study)

Subject signs informed consent to perform required testing for the protocol

Dialysis dependent renal function; In the clinician's opinion, too ill to perform the protocol testing; Pregnant or nursing

Transplant other than heart or lung

In the clinician's opinion, too ill to perform L-PVDOMICS testing even if limited testing.
