L'étude vise à évaluer une approche spécifique appelée le modèle LIFT-ECHO pour améliorer les soins aux patients atteints d'Insuffisance Intestinale Chronique (IIC). Cette condition affecte le système digestif, rendant difficile pour le corps d'absorber correctement les nutriments. La recherche implique des médecins provenant d'au moins 150 sites différents, chacun supervisant des patients atteints d'IIC. En comprenant comment le modèle LIFT-ECHO peut améliorer le traitement, l'étude cherche à offrir de meilleurs résultats de santé pour ces patients, potentiellement conduisant à une meilleure qualité de vie et à une gestion plus efficace de la condition. Dans cet essai, les médecins sont assignés aléatoirement soit à continuer leurs pratiques habituelles, soit à adopter le modèle LIFT-ECHO. Cette affectation est déterminée par un algorithme informatique. Une fois qu'un médecin commence à utiliser le modèle LIFT-ECHO, il continue avec celui-ci tout au long de l'étude. Initialement, il y a une période de 6 mois où les pratiques normales sont observées, suivie d'une introduction de 2 mois du modèle LIFT-ECHO. Après cela, les patients continuent à recevoir des traitements sous la nouvelle approche pendant un an. L'étude examine comment les soins aux patients s'améliorent au fil du temps, aidant à déterminer l'efficacité du modèle dans la gestion de l'IIC.
Inclusion Criteria: * CIF patient participants must meet the following criteria in order to be considered eligible for participation: * Individuals of adult age (18 years or older), including pregnant women or Individuals of age \> 5 years with parental consent and patient assent. * Diagnosis of chronic intestinal failure (ie. on PN \>6 months with no malignancy, no HIV and a malabsorption or post-surgical diagnosis) * Currently receiving PN from a homecare infusion service company * Physician Inclusion Criteria: * Physician participants must meet the following criteria in order to be considered eligible for participation: * Individuals of adult age (18 years or older), including pregnant women * Hold an MD or DO degree * Be a licensed physician prescriber with a homecare infusion service company Exclusion Criteria: * Participants meeting one or more of the following criteria will be excluded from participation: * Adults unable to consent (individuals with impaired decision-making capacity) * Prisoners * Children \> 5 years who dissent
